School physicals

School Physicals: What Florida Requires

Students entering a Florida school for the first time, including kindergarten, need a school-entry health exam done within one year before enrollment.

Sports physicals

Sports Physicals, With An ECG Included

Florida high school sports physicals use the Florida High School Athletic Association (FHSAA) Preparticipation Physical Evaluation, Form EL2. Download the current Form EL2 from the FHSAA and bring it with the student and parent sections filled in and signed.

Alternatively, we can provide the form on arrival to fill out.

The FHSAA form is valid for 365 calendar days from the date of the exam. Under Florida law and FHSAA rules, a completed physical must be on file with the school each year before any practice, tryout, workout, or conditioning. For middle school or league sports, bring the form your school or league uses.

The examining clinician decides whether the student is medically eligible based on the health history and the exam. Some students need follow-up before they’re cleared.

What To Know About The ECG

At our office, every sports physical includes an ECG (electrocardiogram), a quick, painless test that records the heart’s electrical activity. It’s done here, as part of the same visit.

For Florida high school athletes: starting with the 2026–27 school year, students in grades 9–12 taking part in FHSAA high school athletics for the first time need an ECG screening recorded on FHSAA Form EL1. Limited exceptions apply. An ECG done within two years before July 1 of the school year can count — for 2026–27, on or after July 1, 2024. Form EL1 goes to your school. If your student needs it, ask us about it at the visit.

Alternatively, we can provide the form on arrival to fill out.

The ECG is a screening for students without heart symptoms. If your child has chest pain, fainting, a racing heartbeat, or shortness of breath with exercise, tell us — that needs a medical evaluation, not just a screening.

Work physicals

Work Physicals For New And Current Jobs

Starting a new job, or need a physical your employer requires? We do work physicals, including Pulmonary Function Testing ("PFT") as needed, Respiratory Fit Testing, and Drug Testing.

DOT physicals

DOT Physicals For Commercial Drivers

Commercial drivers who need federal medical certification can get their DOT (U.S. Department of Transportation) physical done at Fort Pierce Family Urgent Care.

For commercial driver's license (CDL) holders, the examiner sends your results electronically to the FMCSA, which passes them to your state's driver license office.

Annual physicals

Annual Physicals

An annual physical is a once-a-year look at your overall health, not a visit for one problem. It's a good time to be screened for and talk about ongoing conditions.

What An Annual Physical Covers

  • Your health history and current medicines
  • A physical exam
  • Lab work if the doctor orders it
  • A check-in on ongoing conditions
